Histologic Features
- Ulceration of the skin surface
- Confluent parakeratosis
- A proliferation of atypical keratinocytes emanating from the adnexal structures and interadnexal epithelium, extending to the reticular dermis
- Premature cornification in the form of horn pearls and squamous eddies
- Striking atypia involving the full thickness of the epithelium with many atypical mitotic figures, multinucleated giant cells, bizarre-shaped giant cells, and necrotic keratinocytes
- Destruction of the adnexal structures
- Associated solar keratosis adjacent to the lesion
- Prominent solar elastosis of the dermis
