lichenoid purpura (Gougerot and Blum)
Histologic Features
- Compact orthokeratosis
- Focal spongiosis at the basal layer
- Lymphocytic exocytosis in the epidermis
- Slight epidermal hyperplasia
- Focal vacuolar alterations above the lichenoid infiltrate
- A patchy or band-like lymphohistiocytic infiltrate at the dermo-epidermal junction
- Extensive extravasation of red blood cells
- Melanophages in numbers, especially at the base of the lichenoid infiltrate
- Coarse collagen in the papillary dermis (late stages)
- A superficial and deep perivascular lymphohistiocystic infiltrate
