Histologic Features
- A relatively broad but slightly asymmetrical lesion characterized by
- A proliferation of slightly atypical melanocytes as solitary units and nests at the dermo-epidermal junction beyond the dermal nevus component ("shoulder effect")
- Variation in size and shape of melanocytes
- Confluence of adjacent nevocytic nests
- Persistent lentiginous melanocytic hyperplasia
- Random cytologic atypia
- Irregular spacing of nevocytic nests
- Lamellar fibroplasia in the upper dermis
- Concentric fibroplasia around elongated rete ridges
- Melanin and melanophages with fibrosis in the papillary dermis
- A lymphocytic infiltrate at the base
- Abundant cytoplasm with dusty melanin
